We serve patients in the DFW metroplex and partner with you, your caregivers, and even your primary care doctors while ensuring your unique healthcare needs are met and are in line with your healthcare goals. We are here to improve your health throughout your healthcare journey by providing you with the expertise needed to unlock the path that is right for you.
Our team will connect with you while you are in the hospital to make your transition as smooth as possible. We can be your service provider if you don’t already have a primary care physician (PCP) or if you are unable to visit your PCP.
We will review your long-term care options and coordinate physician appointments in your home to provide acute and post-operative care. Our team of experts will focus on the continuum of care for your chronic illness, so you are not alone on your healthcare journey.
What happens if you are being discharged from the hospital but are not ready to go home? We can help unlock those next steps in your healthcare journey. We will work alongside you and review your options for long-term or acute-care hospitals, inpatient rehabilitation, skilled nursing, and assisted living facilities. We will discuss the fundamental variations between different care facilities and which one best suits your needs.
Palliative care is a resource for patients that provides medical assistance to those who have a serious illness. With palliative care, we would work with you to improve your quality of life, help you understand your choices, and determine the best medical treatments.
Hospice care, similar to palliative care, provides comprehensive comfort care and support for you or your family member in Hospice. Hospice is an approach to care, not tied to a specific place, provided to a person with a terminal illness. Hospice can be offered at home, in the hospital, in a nursing facility, or even in a hospice center. Let us help you choose the right healthcare path for you or your loved one.
